If you wish to register with our GP, please click this link Register with a GP Surgery and complete the form online. After you have completed the online registration, please email us on darlaston.medicalcentre@nhs.net with a valid proof of photo ID and a proof of your current address. Please ensure this is done within 7 days of submitting your online registration to complete the process.
You will then be contacted by the number that you have provided and you will be asked to make an appointment for a new patient check, you will be asked to provide proof of address. You will not be registered until you have attended this appointment. Please note that New Patient Checks are subject to the Nurses availabilty.
